By accessing or using the Deliberate Wellness Studio website and services, you agree to be bound by these Terms of Service. If you do not agree to these terms, please do not use our services. We reserve the right to update these terms at any time, and continued use of our services constitutes acceptance of any changes.
Our wellness and medical services are available to individuals 18 years of age or older. Certain services may require a health screening or medical consultation prior to use. You agree to provide accurate health information and to follow all safety guidelines provided by our staff. We reserve the right to refuse service to anyone for safety reasons.
Appointments can be booked online through our scheduling system or by contacting us directly. We request at least 24 hours notice for cancellations. Late cancellations or no-shows may be subject to a fee. Memberships and punch passes are subject to their own specific terms regarding usage and expiration.
Membership plans are billed on a recurring monthly basis. You may cancel your membership at any time with 30 days written notice. Refunds for unused portions of memberships are not provided. All prices are subject to change with 30 days advance notice to active members. Payment is due at the time of service unless you have an active membership or prepaid package.
Participation in wellness and medical services involves inherent risks. By using our services, you acknowledge and accept these risks. You agree to sign all required waivers and consent forms before participating in services. Deliberate Wellness Studio and its staff shall not be liable for any injuries, damages, or losses arising from your use of our services, except in cases of gross negligence.
Our services are not intended to diagnose, treat, cure, or prevent any disease. Information provided on our website and by our staff is for educational purposes only and should not be considered medical advice. Always consult with a qualified healthcare provider before beginning any new wellness program, especially if you have existing medical conditions.
All content on this website, including text, graphics, logos, images, and software, is the property of Deliberate Wellness Studio and is protected by copyright and trademark laws. You may not reproduce, distribute, or create derivative works from our content without express written permission.
These Terms of Service are governed by the laws of the State of Utah. Any disputes arising from these terms or your use of our services shall be resolved in the courts of Utah County, Utah. If any provision of these terms is found to be unenforceable, the remaining provisions shall continue in full force and effect.
